- Primary Care Nursing Program
The Primary Care Nursing program has been implemented to enhance health promotion and disease prevention. Each physician within the network is allocated three (3) half days of nursing time per month. In a one to one setting, the Primary Care Nurses will provide education and set goals to provide patients with the tools necessary to manage their health. The Primary Care Nurse will be utilized in a wide variety of primary care health needs and will provide a strong knowledge in three (3) care areas:

- Cognitive Impairment
- End of Life
Population Health:
- Smoking Cessation
- Project Homeless Connect
- Weight Management
Chronic Disease Management
- Stages of Change
- Diabetes
- Hypertension

- Mental Health Program
The Primary Care Network is developing a "menu of mental health services" to provide physicians with easily accessible services for mental health patients. Currently the PCN has provided two dedicated services:
Mental Health Program
Two (2) Mental Health Consultants are available at the PCN clinics. Patients can be referred to these Mental Health Consultants who will offer patients brief therapy (less than 10 sessions of 30-60 minutes in length) directed at changing maladaptive thought patterns, behaviours and coping strategies that undermine physical and psychological health. The PCN provides this service at no cost to the patient. Patients are referred to the program through their PCN member physician.
Behavioural Health Program
Behavioural Health Consultants (BHC) are made available to participating PCN physicians. Each BHC can support approximately 20 physicians and will see patients in the physician's office. In 15-30 minute sessions, the BHC will focus on behavioural modification and provide patients with the tools they require to progress. Physician's are eligible for two (2) half days per month of Behavioural Health Clinics.

- In Hospital Program
The Calgary West Central Primary Care Network (CWC PCN) supports an In Hospital Program to help strengthen the continuity of care between CWC PCN Physicians and patients who have been admitted to Rockyview General Hospital. PCN Physicians are available, on a rotational basis, to provide inpatient care for patients of our member physicians. The In Hospital Program is designed to improve the flow of communication and transfer of medical health record information, medication reconciliation and provide timely follow up during patient transitions between the hospital and community.

- Referral and Discharge Planning Program
To assist member physicians in completing complex referrals and discharge planning, CWC PCN implemented the Referral and Discharge Planning Program providing a medical office assistant (Referral and Discharge Coordinator) to the physician's office. The Referral and Discharge Coordinators are Medical Office Assistants who work in the member physicians office to facilitate and support the referral and discharge planning process.

- Physical Therapy Program
The Physical Therapy Program is designed to enhance access to physical therapy treatment for patients who are unable to secure physical therapy.
The Calgary West Central Primary Central Network Physical Therapy Program will compliment the Regional Physical Therapy Program by facilitating treatment for those patients that do not qualify for the Regional Program based on financial need. The program will provide physical therapy treatment for patients who would not otherwise be able to obtain physical therapy. Physicians assess and determine those patients who would benefit from the physical therapy treatments.

- Critical Laboratory Results and Long Term Care Support Program
The Critical Laboratory Results and Long Term Care Support Program provides physician support after hours from 1700 hr to 0800 hr Monday to Friday and all day Saturday, Sunday and statutory holidays. LTC patients/nurses call 1-866-334-1393 to reach the 'After Hours LTC Physician' during these hours.
An additional responsibility of the Long Term Care After Hours Support Physician is to address any critical lab results that come in for Primary Care Network patients after hours. If the patient's family physician is not available to take the critical lab results after hours, the PCN after hours physician (Long Term Care) will address the lab results with the patient.

- Just Like Home Program
The Just Like Home Program offers a welcoming environment where guests receive temporary care from our dedicated nursing staff. The program provides a homelike environment that enables both the guest and the caregivers to get the rest that they need to continue helping their loved one remain healthy and in the comfort of their own home.
The program operates as a day program/overnight accommodation/social club. Regular social and recreational activities are available and include craft making, and stimulating activities. The program allows the care giver to relax, socialize with friends, recover from stress and to recharge and renew to continue to provide quality care giving in the home.
The Just Like Home Program is provided by the Calgary West Central Primary Care Network at a subsidized rate. Guests are charged $ 50 per night.

- After Hours Program
Primary Care Network Clinics
Patients are assessed by HealthLink and the physician (if required) via telephone. If a patient should be seen by a family physician within 24 hours and they do not have a family physician, or they cannot get in to see their family physician, they may be referred to a Primary Care Network Clinic. One physician is assigned to each of the Primary Care Network Clinics. See the Clinic Information page for more information.
Obstetrical After Hours Program
To provide after hours care for Obstetrical minor emergencies the Calgary West Central Primary Care Network has launched an After Hours Obstetrical Program. This program is available to patients of CWC PCN physicians and unattached patients. Primary Care Network Physicians are available after hours, Monday through Friday (1700 hrs to 0800 hrs) and all day (24 hrs) Saturday, Sunday and statutory holidays.
To reach the After Hours Obstetrical Physician for after hours care of obstetrical minor emergencies, patients are directed to call HealthLink at 403-943-5465. The HealthLink Nurse will complete a nursing assessment and triage. If it is determined that the patient is not being followed by an Obstetrician and should be referred to the After Hours Obstetrical Family Physician, the HealthLink Nurse passes the patient name and telephone number to the PCN Call Centre. The Call Centre then contacts the After Hours Obstetrical Family Physician.
After Hours Telephone Support
To provide after hours care for minor emergencies the Calgary West Central Primary Care Network has launched a Telephone Support After Hours Program. This program is available to patients of CWC PCN physicians and unattached patients living in the CWC PCN geographical area. Primary Care Network Physicians are available after hours, Monday through Friday (1700 hrs to 0800 hrs) and all day (24 hrs) Saturday, Sunday and statutory holidays.
To reach the After Hours Physician for care of minor emergencies, patients are directed to call HealthLink at 403-943-5465. The HealthLink Nurse will complete a nursing assessment and triage. If it is determined that the patient should be referred to the After Hours Physician, the HealthLink Nurse passes the patient name and telephone number to the Call Centre. The Call Centre then contacts the After Hours Physician. The Call Centre rotates the calls among each of the After Hours Physicians.

- Primary Care Network Clinics
The Calgary West Central Primary Care Network has established two community family practice clinics to provide primary care services to patients who cannot find a family physician. Each clinic is staffed with a multidisciplinary team consisting of a nurse, support staff, mental health consultant, day time physicians and after hours rotating physicians. This program provides patients with primary care support and provides physicians the opportunity to build their practice with minimal overhead and other expenses.
The Primary Care Network Clinics also support the After Hours Telephone Support Program. The After Hours physician may refer the following patient groups to the Primary Care Network Clinic:
- Patients without a Family Physician
- Patients with a Family Physician that require follow-up within 24 hours but who are unable to see their family physician within that time frame.

The Primary Care Network Clinics provide appointments each day for unattached patients. Primary Care Network staff at the clinics will book appointments for the patients as openings are available.

Specialized Seniors Clinic
The Calgary West Central Primary Care Network has established a Specialized Seniors Clinic to support the high number of unattached seniors requiring complex primary care. The clinic is staffed with a multidisciplinary team consisting of a nurse, support staff, mental health consultant, and seniors care physician. This program provides senior patients with longer appointment times to accommodate the complexity of needs. Primary Care Nursing support is available at the clinic.

- Tsuu T'ina Health Program
The Tsuu T'ina Health Program provides primary care services to residents living in the Aboriginal community Tsuu T'ina Nation. The program is delivered at the Health Centre five half day clinics each week.
The program is supported by a group of physicians that would like to maintain a regular on-going relationship at Tsuu T'ina. Physicians are also requested to participate in a monthly planning meeting to advance the program, and set clinical standards of practice and develop program evaluation.
A Primary Care Nurse is also available at the Health Centre. The Primary Care Nurse works to the full scope of nursing practice offering specialized clinics and support for obstetrical, chronic disease, and mental health management.

- Making Health Happen Program
The Making Health Happen Program is a health promotion strategy that reaches across the Calgary West Central Primary Care Network. The program involves implementation of educational and promotional materials including a new website, a promotional DVD, an educational wallet card library, educational posters and a bimonthly physician newsletter.

The Access Geriatric Health Program provides family physicians, patients and their families with support for management of seniors with complex health needs. Through the Access Geriatric Health Program, family physicians can match the needs of senior patients (and families) with available programs and services, get direct assistance for managing complex geriatric cases including nurse specialist and pharmacy consultation.

The Primary Care Pharmacy Program was implemented to improve patient health outcomes by collaborating with physicians on medication related concerns. The program supports physicians caring for patients with complex pharmacotherapy needs by performing structured medication reviews. In addition, the Primary Care Pharmacist will provide new evidence-based drug information to health care professionals within the practice setting.
