What are Primary Care Networks?

Primary Care Networks are partnerships between a group of local physicians, Alberta Health and Wellness and Alberta Health Services. All three parties work together to build on the strengths and resources that already exist within our health care system to provide comprehensive care to patients.

Calgary West Central Primary Care Network

The Calgary West Central Primary Care Network (CWC PCN) represents a partnership of over 275 local physicians working in over 115 clinics and Alberta Health Services. The CWC PCN became operational in August 2006.
The Calgary West Central Primary Care Network physicians and staff do more than care for the sick and treat disease. We address the unique health needs of our geographical population and implement innovative programs, many of which are prevention focused. The needs for Calgary West Central have been identified as:
  • Services for seniors
  • Care for mental health concerns
  • Management of chronic diseases
  • Care for minor emergencies

Vision:

Expanding capacity and improving patient access to Primary Care through physician support and multidisciplinary teams.

Objectives:

Success for the Calgary West Central Primary Care Network will include meeting the following objectives:
  1. Better coordination of patients throughout the health care system.
  2. Better management of patients with chronic and complex conditions.
  3. Improved health promotion education and screening for patients.
  4. Better coordination of care for patients with mental health issues.
  5. Increased number of physicians enrolled in the Primary Care Network.
  6. Improved recruitment and retention of family physicians and other health care professionals within the Primary Care Network.
  7. Improved support for a coordinator of health care management for seniors.

Geographical Area

The Calgary West Central PCN covers a geographic area within the Calgary city limits bounded on the north and east by the Bow River, on the south by Anderson Road and on the west by the city boundary. This geography takes in some of the most affluent, as well as some of the most deprived, areas of the city, including the downtown core.
